Elderly People at Risk of Losing their Independence
Issues
Improving the management of over-75s who are still independent, but whose health is likely to deteriorate.
The PAERPA approach envisions acting prior to hospitalisation by improving primary care and maintaining elderly people’s independence through better cooperation between professionals.
Solution
The DO-Pill smart pill dispenser allows us to:
– Reassure and assist patients who have difficulty taking their medication as prescribed
– Make sure medication is taken as prescribed
– Optimise correct use of medication and act against its side effects
5 Rs rule (Right medication, at the Right dose, at the Right time, administered in the Right way and to the Right patient)
Results
31 pharmacies trained:
– 63.63% of pharmacists were very happy with the training received. The others were happy with the training received.
– Over 80% of pharmacists find it very easy to use DoSup, the monitoring and configuration software for DoPill.
– In total, over 30 patients have used a DO-Pill
– The average compliance rate is 98%

Mentally Handicapped Adults and Children
Issues
Monitoring patients in a home for handicapped adults: Résidence du Parc in Malzéville.
This home provides accommodation and care to handicapped adults who work during the day, in an ordinary working environment, vocational rehabilitation centre (ESAT) or disability-friendly company.
Solution
The DO-Pill smart pill dispenser allows us to:

ETP Pilot is of interest to all organisations that manage or coordinate patients, providing payers with the reporting items required to justify their business.
ETP Pilot can be used by organisations with a TPE programme as well as those wishing to coordinate support programmes (pharmaceutical laboratories, mutual funds, medtech companies, telephone providers, etc.).
Solution
ETP Pilot is a web application that monitors therapeutic education courses.
This solution facilitates the work of coordination teams at these organisations who are faced with demands for quality management, productivity and traceability.

E-Nephro study
Management of chronic kidney disease
Issues
A system for managing patients with chronic kidney disease at all stages (chronic kidney disease, dialysis patients, kidney transplant patients).
Objectives:
– To improve the patient’s health and quality of life.
– To reduce management costs especially in terms of unplanned hospitalisations and consultations
Solution
It is based on tools developed 10 years ago in Lorraine by Diatélic for the Néphrolor (managing patients in the early stages) and Altir (managing dialysis patients) networks, which will be improved and rolled out in two other regions: the North, and Aquitaine.
